Failure Action Effectiveness Chart: Purpose and application
The Failure Action Effectiveness Chart is a spreadsheet that documents warranty claims from a specific production period. It includes:
– A description of corrective actions (both interim corrective actions – ICA and permanent corrective actions – PCA) implemented for each type of problem (failure mode).
– Planned activity dates, if no actions have yet been taken, providing both the supplier and the customer with visibility on when the problem is expected to be resolved.
– Verification of the effectiveness of ICA and PCA actions, allowing for consistent monitoring and updates.
Additionally, the chart can include the number of warranty claims for a product or product group, categorized by problem type or cause, and organized by production date.
Automotive Customer Requirements
The first reference to the use of this chart is found in the VDA FFA manual, which is utilized by customers such as the Volkswagen Group (VW). It’s important to note that if a corporate template exists, its use should be discussed and agreed upon with the customer representative.
The second reference is the GS 95004 standard used by BMW, specifically in Annex 3.
The third customer is Daimler, which mentions the Failure Action Effectiveness Chart in the MBN 10448 standard.Daimler also refers to this document as the Payner Chart, using it to manage reports from the warranty field.
